Job Title: Primary Care Integration Development – Service Coordinator (4x posts, 1 in each commissioning locality across Sheffield)
Salary: Circa £40k (pro rata for part time)
Contract type: Fixed term contract (6-12 months) -secondment opportunity
Base/Location: To be agreed – it will be in primary care setting
Responsible to: PCS Project Manager

Primary Care Sheffield (PCS) is a new organisation that is owned and run by GPs in Sheffield. It is a company that has been formed to ensure that primary care has a strong voice in the development of the health and social care within Sheffield. It has been established to secure and run contracts for health and social care in the city. PCS has already secured its first major contract to deliver a Prime Ministers Challenge Fund Programme.
